The following must be completed prior to enrollment
Please bring to registration:
Birth Certificate
Michigan Law requires that the school maintain a copy of each student's birth certificate.
Record of Immunization
In order for your student to enter school, proof of current immunizations must be submitted as required by the Michigan Department of Public Health.
Proof of Residency/ Residency Affidavit
Documentation must be submitted with registration materials and affidavit signed.
Residency affidavit is available at the District Office at Union School
If applicable:
7th - 12th Grade Students
The counseling office needs to be contacted for an appointment.
High School - 429-8034
Middle School - 429-8070
To aid in scheduling classes, please bring a copy of your student's latest report card.
Proof of Guardianship
If the student is not living with either parent, proof of full guardianship from the Probate Court must be submitted. This document must be updated annually.
Court Documents
A parent who wishes to have the school limit access to the child by the other parent must provide legal documentation to this effect at the time of registration. The school cannot deny a natural parent access to his/her child without documentation on file.
